What to Expect on Your 1st Visit

• You will need to fill out a questionnaire that includes health history, dental history and dental insurance.
• You will need to bring your Driver's License and/or another form of picture I.D. and any dental insurance cards you may have.
• You will need to provide a list of any prescription and/or nonprescription medications you are taking.
• During your appointment we will:
  • Address any concerns that you might have
  • Take any necessary x-rays
  • Prepare a treatment plan
  • Do a prophy (cleaning) if time allows
  • Do a full exam including:
    • cavity check
    • periodontal (gum) disease check
    • oral cancer screening
    • TMJ Evaluation
    • orthodontic evaluation
    • consultation/recommendation
